Insurance
​
Jody M. Leach, Ph.D. & Associates is a Private Pay practice, therefore is Out of Network on all insurance panels. Services are provided and charged directly to the client, not to the insurance company. You are responsible for full payment of fees.
​
We will provide you with an invoice (also known as a “superbill”), which has all the necessary information for you to bill your insurance company for reimbursement, if eligible. You pay the full fee upfront and receive a percentage of it back, as determined by your insurance.
​
Prior to beginning therapy, it is important that you contact your insurance administrator to verify which mental health services, if any, are eligible for reimbursement, with your particular policy, etc.
​
In order to get an accurate idea of your out-of-network insurance benefits, here are some questions you can ask your insurance provider. Call the phone number on your insurance card and ask:

"What percentage am I reimbursed for routine behavioral health care out of network"?

"What is my deductible?" Find out if you have a deductible for out of network providers and how much it is.

"How much of my deductible have I already met?"
-
“Is there an “allowable amount” for behavioral healthcare (this is sometimes referred to as “reasonable and customary fees”)?

If you need us to provide you with procedure codes, we can help you with that.
With the above information, you should be able to know what your out of pocket expense will be.

Cancellation Policy​
Your session is reserved specifically for you.
We have a strict 24-business-hours policy regarding canceling or rescheduling appointments.
If clients are unable to attend their scheduled visit, they must contact us at least 1-business day in advance to avoid getting charged the full amount of the visit.
Our client system sends a reminder email 48 hours prior to appointments however, these reminders do not exempt payment for missed appointments.
Please note that insurance companies do not reimburse charges for missed appointments, which means the cost will be the sole responsibility of the patient / responsible party for billing.

Good Faith Estimate of Services
​
As of January 1, 2022, the “No Surprises Act” gives you the right to receive a “Good Faith Estimate” of your mental health care costs. Under this law, health care providers need to give clients who either don’t have insurance, or are not using insurance, an estimate of the expected charges for consultations and comprehensive psychological/psychoeducational evaluation services. We will discuss the fees with you, and you can ask us for a Good Faith Estimate before you schedule your first session. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
